Participant Waiver, Release of Liability, and Parent/Guardian Consent

1. Acknowledgment of Risk

I, the undersigned parent or legal guardian of the above-named participant, understand that participation in soccer training, conditioning, camps, clinics, games, tournaments, and related athletic activities organized by FC Stars United LLC involves inherent risks of injury.

These risks may include, but are not limited to:

• Falls
• Collisions with players, equipment, or surfaces
• Sprains, strains, fractures, or concussions
• Heat-related illness
• Serious bodily injury
• Permanent disability
• Death

I voluntarily allow my child to participate with full knowledge and acceptance of these risks.

2. Release and Waiver of Liability

In consideration for participation in FC Stars United LLC programs and activities, I, on behalf of myself, my child, and our heirs, executors, representatives, and assigns, hereby release, waive, discharge, and hold harmless FC Stars United LLC, its owners, officers, directors, employees, coaches, trainers, volunteers, contractors, affiliates, sponsors, facility owners, and agents from any and all claims, liabilities, demands, damages, causes of action, costs, or expenses arising out of or related to participation in any FC Stars United LLC activity, including those arising from ordinary negligence.

This waiver applies to:

• Personal injury
• Illness
• Property damage
• Medical expenses
• Loss of income
• Death or disability

except where prohibited by applicable law.

3. Medical Authorization

I certify that my child is physically able to participate in athletic activities and has no medical condition that would prevent safe participation unless disclosed in writing below.

In the event of an emergency, I authorize FC Stars United LLC staff, coaches, or representatives to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am financially responsible for any medical treatment provided.

4. Parent/Guardian Responsibilities

I agree to:

• Ensure my child follows all rules, instructions, and safety guidelines
• Provide accurate medical and emergency information
• Inform FC Stars United LLC of any medical concerns or restrictions
• Accept responsibility for my child’s behavior and conduct

FC Stars United LLC reserves the right to remove any participant whose behavior is unsafe, disruptive, abusive, or inappropriate without refund.

5. Media Release

I grant permission to FC Stars United LLC to use photographs, video recordings, or images of my child taken during training sessions, camps, events, or activities for promotional purposes, including:

• Social media
• Website content
• Advertising materials
• Printed marketing materials

6. Privacy Acknowledgment

I acknowledge that I have reviewed and understand the FC Stars United LLC Privacy Policy and consent to the collection and use of personal information as described therein.

7. Governing Law

This Agreement shall be governed by and interpreted under the laws of the State of Illinois.

If any portion of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

8. Acknowledgment

I HAVE CAREFULLY READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.
