Consent and Authorization

I, the undersigned parent or legal guardian of the above-named participant, hereby acknowledge and agree that FC Stars United LLC may collect, use, store, and disclose certain personal information relating to my child for purposes associated with participation in training programs, camps, events, and related activities.

I understand that the information collected may include, but is not limited to:

• Player and parent/guardian contact information
• Date of birth and age verification
• Emergency contact information
• Medical or allergy information voluntarily provided for safety purposes
• Attendance and participation records
• Photographs and video recordings taken during training sessions or events
• Payment and registration information

Purpose of Information Collection

I understand that the information collected may be used for:

• Program registration and administration
• Communication regarding schedules, updates, and emergencies
• Safety and medical response purposes
• Player development and training operations
• Internal business and recordkeeping purposes
• Marketing and promotional materials, including website and social media content

Disclosure of Information

I understand that personal information may be shared with:

• Authorized coaches, trainers, and staff members
• Third-party service providers assisting with registration, payment processing, scheduling, communication, or data storage
• Medical personnel or emergency responders when necessary

FC Stars United LLC does not sell personal information to third parties.

Media Release Consent

By signing below, I grant permission to FC Stars United LLC to photograph or record my child during training sessions, camps, games, and events for promotional and marketing purposes, including:

• Social media platforms
• Website content
• Printed promotional materials
• Advertising and marketing campaigns

Acknowledgment

I acknowledge that I have reviewed and understand the FC Stars United LLC Privacy Policy and consent to the collection and use of my child’s information as described therein.

I understand that I may request updates or corrections to personal information and may withdraw certain consents in writing where permitted by law.
